Follow these 4 simple steps to request quilting by mail:

 

Click Button 1:

1 - COMPLETE A SERVICE REQUEST FORM

Complete the Service Request Form and print two copies. One copy will accompany your quilt and the other is for your records.

 

Click Button 2:

 2 - PACKAGE YOUR QUILT FOR MAILING

Review the Quilt Prep instructions. Package your quilt top along with a copy of the completed Service Request Form. See mailing address on the form.

 

Click Button 3:

 3 - MAKE 50% DEPOSIT PAYMENT

A deposit of 50% is due before work begins on your quilt top. This payment will alert me that your quilt is on its way. See the Deposit Amount on the Service Request Form. Click this button to pay that amount with your credit card or pay using PayPal.

You will receive an e-mail when your quilt is received. I'll provide an estimated completion date and ask any last questions before work begins.

 

Click Button 4:

4 - MAKE FINAL PAYMENT

You will receive an e-mail when your quilt is completed. Final payment is due at this time. This payment will trigger shipment of your completed quilt to you. See the Final Amount on the Service Request Form. Click this button to pay this amount with your credit card or pay using PayPal.
